I have the "Alert - New System" report turned on, but every time I acknowledge a change I get this error.

First try deleting the report and reactivating to see if that helps.

Do you have any errors in your php_error file that might shed some light on the situation?

I don't have any PHP errors on the log. I'm using 5.2.11 (if that makes a difference), SQL logs are clean too.

I do have have an error log on Apache related to favicon.ico which is completly irrelevant.

I'm surprised nobody else has mentioned anything about this error. I'm attaching a screenshot just to make it evident. [img]http://www2.bc.edu/~mendozaa/temp/oa2error.jpg[/img]

That Ack Change form sends you to the "Installed Software" report after submit.
Make sure you have that report activated.
